1-The College applicant can fill out the Application Form.

2-The applicant’s physician must fill out the Medical Form.

3-The applicant’s pastor must fill out the Pastoral Recommendation Form.

4-The General Recommendation Form must be filled out by an employer, teacher, deacon or someone other than a family member.
